Name: Gorton, Calum (60460) Licence category: 1.4.230 Signalling Maintainer & Fault Finder. Date of revocation 23.03.21. Details of complaint: Calum Gorton completed record cards, together with servicing and FPL testing records for work on points. These incorrectly showed the work and testing as having been completed.
Name: Pickstock, Graeme (0070589) Licence category: 1.4.230 Signalling Maintainer & Fault Finder. Date of revocation 06.08.20. Details of complaint: A code 2 AWS fault occurred at signal PN315 (horn vice bell). A fault team attended and found wiring defects in location HH19/3. Graeme Pickstock had been responsible for the work in accordance with a SMTH test plan list issued by the Section Supervisor prior to the work. The investigation found that the work had not been satisfactorily tested and that Graeme Pickstock had carried out both the installation and testing work in contravention of the independence requirements of SMTH. The subsequent investigation found that Graeme Pickstock had failed to maintain independence between installation and testing, completing both himself. The installation had not been satisfactorily tested and had been returned to operational service with incorrectly run, unterminated and uninsulated wires, some of which were live and left hanging unrestrained.
Name: Turmaine, Kelly (0069018) Licence category: 1.4.230 Signalling Maintainer & Fault Finder. Date of revocation: 05/05/2022.
Details of complaint: During a failure investigation a faulty cable core was identified and a diversion carried out. the diversion and SMTH testing in 1 of the locations was carried out by Kelly. The diversion was carried out incorrectly and fitted to the wrong side of a bind post which resulted in a bypassed track relay contact within the circuit and the potential for a wrong side failure.
